Below, you'll find an overview of our onboarding process and a checklist of items needed to complete your employee profile:

Employment Application - Complete and sign our standard employment application form.
W4 Form - Complete and submit your federal tax withholding information.
I9 Form - Complete and provide documentation to verify your eligibility to work in the United States.                           AHCA Background Screening (Level II) - Required for positions working in human services sector.
Local Background Screening - Obtain from your local sheriff or police station.
Government-issued ID - Driver's license, passport, or state ID.
Social Security Card - Ensure your SSN is valid and up-to-date.
Resume - Highlighting your relevant skills and experience.
Proof of Education - High school diploma, college degree, or transcripts (official copies may be required).
Auto Insurance Declaration Page (Admin Only) - If applicable to your role.
Auto Registration (Admin Only) - If applicable to your role.
Affidavit of Good Moral Character - Sign and submit as part of the background check process.
Two References - Professional or personal references who can speak to your character and work ethic.
Professional License or Training Certifications - If applicable to your role (e.g., HHA, nursing license).
Mandatory Trainings - Complete required trainings, including Zero Tolerance, HIPAA, Direct Core Competencies, and Basic Medication Administration.

Once everything is in order, submit your paperwork for processing, and our team will handle the rest.
